Setup or replace a printer

Use this form to request the setup of a new printer in a TMG 'PMS Only' Medtech Deployment.

This is required to determine the eligibility to request changes, and where updates will be sent.
When does this change need to be performed by?
This name should reflect the printer's role - if it was in consult room 4 for example, it could be called 'consult4'.
Examples could be: "Brother HL-6200DN" or "Xerox imageCLASS LBP351x"
All medtech cloud printers must be connect to the network, and must have static IP addresses set in the device, or assigned by the router.
This is required if we are to assign an IP to the printer from DHCP; it is also useful for troubleshooting.
