New User Form

Use this form to request the setup of a new staff member in your practice's computer systems

This is required to determine the eligibility to create new users and is where progress updates will be sent.
It is important to be accurate here; if the Staff's name is incorrect it can be difficult to correct in the IT systems. Generic names such as 'Reception' or 'Consult' will not be accepted.
If the user does not match any of the listed roles, please select 'other' and fill in the field below.
This is used to setup integrations such as email signatures and other applications that integrate in the Practice's active directory. A phone number is not required if they will not be assigned one.
This may include payroll / accounting services or access to another clinic.
Does the user need to be added to any email distribution groups? If yes, please list groups above.
Does the user need access to any shared mailboxes? If yes, please list mailboxes above.
